Enrollment Changes

 

Cancel Participation in the TSA Program

 

1.

Complete a new Salary Reduction Agreement (SRA) reflecting your desire to discontinue Program participation.  The SRA may be obtained from the Employee Benefits Unit or on the Forms page.  The SRA should be mailed to the Employee Benefits Unit.

 

Change Contribution Amount

 

1.

Complete a new Salary Reduction Agreement (SRA) reflecting the change of your contribution amount.  The SRA may be obtained from the Employee Benefits Unit or on the Forms page.  The SRA should be mailed to the Employee Benefits Unit.

 

Change Investment Provider

 

1.

Select the new investment provider or insurance carrier with whom you will maintain your TSA account.  A list of approved providers may be obtained from the Employee Benefits Unit or on the Investment Options page.

 

 

2.

Contact the new investment provider to establish a TSA account.  Your financial advisor or agent may assist you in this process.  Your account must be established with the new investment provider prior to making a change to your Salary Reduction Agreement (SRA).

 

 

3.

Complete a new Salary Reduction Agreement (SRA) reflecting the reallocation of your contribution amount to your new investment provider.  The SRA may be obtained from the Employee Benefits Unit or on the Forms page.  The SRA should be mailed to the Employee Benefits Unit.
